Please let me know if there is any particular ceremony to formalise adoption of a child and details of the same. Also, if the birthdate of the child is known, is there any concept the child's horoscope with that of the prospective parents?

yes there are few formal adoption procedures available in the sastras. propably if the biological parents are still alive there are some procedures to do and also as per my understanding you have to re perform the naming ceremony again. sorry i dont have an answer to your horoscope question. please do make good consultation with family elders and aathu vadhyar before any decision.

Whether the child is of same Gothram or other Gothram can also
be verified before performing adoption procedure. In one of our
relatives place, they did Ayushya Homam and Navagraha Homam
after obtaining the parents' consent and they offered the child
for adoption in the presence of elders. Namakaranam was done
afresh.
